Leland Athletic Handbook - Before a student-athlete is allowed to participate in athletics at Leland Public School they as well as their parents / guardian will need to read and agree to follow the Leland Athletic Handbook by signing a copy of the Athletic Code of Conduct which must be on file before the first practice in the Leland Athletic Office.
Leland Athletic Code of Conduct - Before a student-athlete is allowed to practice a signed copy of the Athletic Code of Conduct by both the parents / guardian and student-athlete must be on file in the Leland Athletic Office. This form may be turned into the school athletic office prior to the first day of practice, or may be given to the coach at the first practice.
MHSAA Physical Form - Before a student-athlete is allowed to practice, he/she must have a completed, physician-signed MHSAA physical card on file in the Leland Athletic Office. Forms are available in the school office or online at MHSAA.com or by clicking the link on this page. Co-op program athletes must have a copy of their physical card on file at both the cooperative school’s athletic office and the Leland High School Athletic Office.
Pay to Participate Fee Guidelines & Procedures - The Leland Public School Board of Education has instituted a fee for participation in interscholastic sports. This fee is designed to generate funds to help supplement the athletic budget by offsetting the costs associated with operating our athletic programs, and must be paid prior to a student-athlete being allowed to practice.
Concussion Awareness Acknowledgement - Before a student athlete is allowed to practice he/she must have received and reviewed the Concussion Awareness Information, which is listed below, and a signed copy of the Concussion Awareness Acknowledgement Form must be on file in the Leland Athletic Office.
Concussion Awareness Information - Before a student athlete is allowed to practice he/she and their parents / guardian must review the Concussion Awareness Information, and keep it in a location for future reference. After reviewing the Concussion Awareness Information, the Concussion Awareness Acknowledgement Form, which is listed above, must be signed by both parent / guardian and student-athlete and then turned into the Leland Athletic Office.
MHSAA Return to Activity / Post Concussion Consent Form - This form is to be used when a student-athlete is removed from an activity after exhibiting concussion symptoms.
Leland Athletics Transportation Variance Waiver Form - The Leland Athletic Handbook requires that student-athletes ride with school provided transportation to and from all athletic events and any variance from this requirement will require parent permission by completing and signing the Leland Athletics Transportation Variance Waiver Form.
Cooperative Sports Private Transportation Permission and Release Form - Participation in cooperative sports requires families of student-athletes to provide transportation to all cooperative practices, contests, and events when not provided by the host school in the cooperative agreement. The Cooperative Sports Private Transportation and Permission and Release Form must be completed, signed, and turned into the Leland Athletic Office prior to the start of the cooperative sports season.
Dual Sports - Student-athletes interested in participating in dual sports during the school year must submit their request in writing to the athletic director prior to the start of the athletic season. Student-athletes are required to meet with both coaches and the athletic director to discuss the possibility of participating in dual sports. This meeting is a requirement prior to participation. Northwest Conference Constitution - The purpose of the Northwest Conference is to promote good sportsmanship, assist in the proper administration of athletic programs, and facilitate scheduling of athletic contests among member schools.
